Chronic Disease Management Plans
If you are managing a chronic or complex condition, you may be eligible for a GP Management Plan (Medicare Item 721) and Team Care Arrangement (Item 723). These allow GP referrals to eligible allied health providers, with Medicare covering up to five allied health visits per calendar year.
